A Psychiatric Study of Clinical Status of Male Sexual Dysfunctions


Abstract
In recent years, an interest in sex therapy has been increased in this country. However there is not yet a systematic work on the overall clinical aspects of the sexual dysfunctions in sufficient number of cases. According to the present study, the author has attempted to make clear the actual clinical states of the male sexual dysfunctions of 114 patients who have visited to neuropsychiatric department of kyungpook National University Hospital and Daegu Dongsan Presbyterian Hospital during the period from January 1967 to August 1980.
For the purpose of comparison the patients could be devided into 3 groups on the basis of their case histories: Primary impotence (54 cases), secondary impotence (48 cases) and premature ejaculation (15 cases).
The results could be summarized as follows
1. The male sexual dysfunctions occupied only 0.48% of the total out-patients of the same period.
The partial impotence was most frequent for main symptom, For the most part, they were able to.get erection at masturbation. They most visited hospital within 1 year after the development of the symptom. The more duration prolonged, the less visited. The mean age of them was 30.3 year. Above 65.80 of them has experienced premarital intercourse. The mean age of patients who have experienced first intercourse was 22.7 year, The number of the patients increased more at rural area, first son and agriculture or labour among the total psychiatric patients. They had lower level of educational background and more often were parental loss, compared with total paychiatric patients.
2. Primary impotence group was among youngest, and had higher rate of city residency, higher level of educational background, lower rate of marriage, and shorter duration of marital life among 3 grougs.
They had lower interest in the opposite sex before marriage, more sexual inferiority. honeymoon impotence, sexual ignorance, mental retardation than other grougs.
They visited hospital passively and had no good therapeutic response.
3. Secondary impotence group was older in age and had lower rate of educational background,
rate of marriage and longer duration of marital life than the primary impotence group. They had shorter duration of symptom, highest rate of rural residency and agriculture in occupation among the three groups.
They had higher interest in sex before dysfunction, higher rate of neuroticism and other physical disorder, and they had been more influenced by external factors. They visited more at their own will and had best prognosis.
4. Premature ejaculation group was oldest in age, had higher rate of first son in sibling order, lowest level of educational background highest rate of labor in occupation, highest rate of marriage, longest duration of marital life. Psychologically they had much ignorance and underlying hostility to the opposite sex, much inferiority, and active dominating female as sex partner. They visited hospital most passively and had worse therapeutic outcome.
